Mercy Medical Center - Mason City / New Hampton

CHANGES IN REFERRAL PROCESS FOR MIDLANDS CHOICE - Providers/facilities in Rochester, MN or associated with the Mayo Clinc and the University of Iowa Hospitals and Clinics are paid at the non-network level, unless an approved referral has been obtained through Aetna prior to having services at one of these facilities/providers.

Mercy Medical Center associates will be required to obtain an approved referral prior to having services at Rochester MN, University of Iowa Hospitals/Clinics, and the Mayo Clinic.

Please make sure your physician is aware: A procedure or service that is available or can be performed at Mercy Medical Center - North Iowa or another network facility will result in non-network benefit reimbursement for a facility/clinic charge at the University of Iowa Hospital/Clinics, Rochester MN, and the Mayo Clinic.

If an associate or family member is referred to the University of Iowa, Rochester MN or the Mayo Clinic for services, the referring physician will need to send a referral to Aetna prior to services being obtained. Referrals for emergency situations will be reviewed upon receipt at Aetna (except for weekends or off hours). Non-emergency referrals will be reviewed within 48 hours of receipt at Aetna.

Referrals approved for the University of Iowa Hospitals/Clinics, Rochester MN or the Mayo Clinic will be paid at the in-network or PPO level. If a referral is not approved and the employee/family member still chooses to have services at the University of Iowa Hospitals/Clinics, Rochster MN, or the Mayo Clinic, these services will be paid at the non-network or non-PPO level.

You may obtain a Transition of Care form by visiting www.aetna.com or by contacting Aetna directly at 1-800-544-5108.

For additional information on the referral process, please contact Aetna at 1-800-544-5108. Again - an approved referral is required prior to obtaining services at the University of Iowa Hospital/Clinics and the Mayo Clinic. Failure to obtain an approved referral from Aetna will result in benefits being paid at the non-network or non-PPO level.
