Flexible Spending Accounts Summary Plan Description

2014 Benefit Plan Year:

A Flexible Spending Account (FSA) gives you a way to pay for your out-of-pocket health care and /or dependent care expenses with before-tax dollars.

Here is how it works: with an FSA, each paycheck you set aside some of your pay, before taxes, to use for eligible expenses. That way you have money set aside for various items not covered by your insurance. By not paying taxes on that money, you can save up to 40 percent on eligible out-of-pocket health and dependent care expenses.

You can use a variety of payment options to access your FSA savings. These include the WageWorks Health Card; Pay my Provider, Pay me Back; or by using the Mobile application. For more information on these options, please go to www.wageworks.com.

Health Care Spending:

You have the option to contribute before-tax dollars in any amount between $130 and $2,500 annually. You can use those savings to pay for eligible health care products and services used by you and/or your eligible dependents. The expenses include items such as:

Vision care, including eyeglasses, contact lenses and saline solution
Dental care, both preventive and restorative
Orthodontia
Physical therapy, counseling, or psychological services

Prescribed Over-the-Counter (OTC) medications
